Celebrity dermatologist Doctor Robert Anolik weighs in on the insider secret that keeps JLo looking so young: THERMAGE.

On the heels of the Super Bowl, the searchterm “how old is JLo?” skyrocketed. The answer? 50. The follow up question? How? We speak exclusively to Doctor Robert Anolik on Thermage, the treatment that keeps the actress, singer, dancer and all-around megastar looking so youthful.

Word on the street is that JLo undergoes Thermage treatment. Can you tell us a bit about the procedure?:

There’s a growing area in cosmetic dermatology of trying to firm, tighten or lift skin without surgery. A lot of the options that exist are usually faceliefts and buttockslifts and armlifts, which all have a lot of downtime, but the benefit of those surgeries is dramatic change. Those are great options, but it’s just about what people need or can do given their lifestyle. Thermage is based on a technology called radiofrequency, and the reason it is one of the popular options for trying to tighten the skin is that radiofrequency essentially delivers an electric current through the epidermis and dermis and that creates a generalized heat throughout those areas. This triggers a process where the skin tightens and develops new collagen—not just at the very surface, but throughout that entire area of the epidermis and dermis. And this can create a tightening effect. We helped develop the first versions of this treatment here at our office, and this started nearly 20 years ago. We did a lot of the early clinical trials, and it has continued to evolve over the past many years to be in its current form. Thermage itself has gone through several evolutions, not just in how it’s treated, but also the technology itself. Initially, at our offices when we were working to develop it, we were using very high energies with very few passes over the skin, but we learned that was more painful and less effective. By gradually- over time – realizing that we can instead give lower energies, which is much more comfortable and no longer painful, and doing it with many passes over the skin we can achieve better results.

Yes, I heard that it was painful but it makes sense that over the years dermatologists would try to limit pain and make things more comfortable for the patient.

People don’t complain of pain anymore with this technology. The device itself, of course, gets modified and upgraded just like everyone’s iPhone does over the years and we’ve brought in the latest system over the past year, which does have a higher peak power, and it has a better cooling mechanism on the handpiece that helps cool the skin and make for greater comfort. It’s not to say that people won’t feel it—the general feedback we get from patients is a hot sensation, but definitely not painful. So we do take that metric and feedback to adjust how we treat each individual patient because that tells us when we’re at the right levels.

How effective is the process? Do people need to go back for follow ups?

Most of our patients who like Thermage will continue to do follow ups because we’re continuing to age constantly, of course. I do find that a lot of my patients who do Thermage are coming in twice a year to have a session, but this is going to be variable based on how great of a responder someone might be. Some people might do it three times a year, while some people might do it one time a year. And there will always be a percentage of people who won’t respond, and they might not continue to do the treatment. But the good news for everybody is that given what we know from the trials, even if we don’t see a visible lifting or tightening in the minority, those people will still likely get a prevention over time because those collagen fibers have been improved and the rate of laxity that is coming should be diminished.

Right. And what is the range in terms of cost of this treatment for Thermage?

It really depends on the body area. To treat a face, it could probably range anywhere between $1,000 and $5,000 to be realistic.

What else can people do in tandem? Do you think Thermage is a one stop shop for patients?

Do I think that people only do Thermage when they’re trying to make themselves have their optimal appearance? Absolutely not. In general, I use so many strategies to make sure our patients look beautiful and natural, and one area of concern is the laxity of the skin. That’s when I recommend to patients to think about Thermage—if they need it. If someone has buoyant, firm skin then they don’t have to think about it, but if you’re addressing laxity, often times Thermage comes into the conversation. Then, if people have discoloration on their skin, they need to use lasers to address those issues. And then if someone has wrinkles and creases around their eyes, they’re going to need to use Botox. If someone has deflated volume on their cheek and chin area, then they’re going to have to use filler. And then, of course, if there is tremendous laxity, they’re going to need surgery. I always tell my patients that when they want to take their best strategy to create their best outcome that there are four main approaches to optimizing cosmetic appearance: surgery when needed, Botox when needed, fillers when needed and energy technology when needed—this is one of the energy technologies. And without all four you won’t optimize, but you may not need all four. It’s just about using them properly. The reason I say that is because a facelift would never replace what Botox can do. And a filler won’t replace what a laser can do and vice versa. They all do different things so well, and when you use all of them when they’re needed you really get a great outcome.
